This piece includes the phrase "gone but never forgotten" along with an outline heart and dates (if you choose to have them), onto a 3/4" -- roughly nickel size -- highly polished stainless steel disc (or 14k rose gold plated or 14k gold plated disc).
You can select the option to have actual handwriting engraved onto the back of the piece as well.

The "as pictured" option includes the handwriting charm, the cubic zirconia cross charm, the angel wing charm as well as the bezel set birthstone charm (you can choose the stone month).
